Rochester, NY -- Hillside-Catholic Charities Behavioral Health Care Collaborative announced a partnership with Unite Us, a leading care-coordination platform integrating healthcare and social services. The Hillside-Catholic Charities Behavioral Health Care Collaborative is one of 19 Behavioral Health Care Collaboratives across the state that has received funding as part of the ongoing reform efforts of New York State’s Medicaid system. This program, in particular, supports infrastructure changes to help providers of mental health and substance use disorder services transition to a business model of value-based payment.
This Collaborative selected Unite Us as the infrastructure to connect the network of providers spanning mental health and substance use treatment systems (behavioral health), physical health, and social determinants of health while capturing outcome data to report back to key program stakeholders.
All partners in the network will be centrally connected to each other, with the ability to:
Electronically connect patients to both clinical and social service providers;
Track patient progress and receive automated feedback from partners to ensure care and services are received;
Collaborate with community-wide “care teams” ensuring providers are on the same page regarding their shared patients’ progress; and
Collect data including structured patient outcomes to measure the network’s impact, including time-to-service and effectiveness of care.
