New York, NYJoining the ranks of healthcare’s most creative and disruptive leaders, Dan Brillman, co-founder and CEO of Unite Us, was selected to speak on a panel at the HLTH Conference to discuss how Unite Us’ unique combination of intuitive technology and comprehensive community engagement process is turning the traditional care delivery system on its head. Over 6,000 senior industry leaders will come together from October 27 to October 30th in Las Vegas to explore promising practices and consider creative improvements to the current healthcare system.

Brillman will share the stage on Sunday, October 27th with Alisahah Cole of Atrium Health and Rich Roth of Common Spirit Health for robust dialogue, on a panel moderated by Elizabeth M. Wroe, Principal with Leavitt Partners. The panel topic is described below:

Correlations between health status and social determinants are well-known but identifying and connecting individuals to their respective need for housing, nutrition, employment, or companionship can be difficult. These innovators are utilizing sophisticated platforms powered with real-time data and analytics to make these correlations sooner - and with greater accuracy! Driven by purpose, devoted by passion and powered by technology, these presenters know it’s cool to live out the Golden Rule.

Recently, Brillman was named to Business Insider’s list of 30 Leaders Under 40 Transforming Healthcare. Together with President Taylor Justice, Brillman founded Unite Us in 2013 to help military friends and colleagues navigate health and social services as they transitioned back to civilian life. No longer just for veterans, today’s Unite Us has 55 community networks in 23 states with more in the pipeline every day. Unite Us’ intuitive platform supports meaningful collaboration, community-wide care coordination, and secure, bidirectional data sharing to address all determinants of health.
