Press Release: CyncHealth, United Way of the Midlands 2-1-1, and Unite Us Improve Access to Care for Nebraskans

Nebraska residents can now self-refer to meet their health and social needs, connecting to over 4000 resources.

Greater Omaha-Council Bluffs, NE (August 2, 2022) – To address the health and social needs of all Nebraskans, the Unite Nebraska coordinated care network now allows self-referrals directly from community members connecting them with over 4000 health and social agencies, programs, and locations across the state with support from a live 2-1-1 Navigator in the Unite Nebraska Coordination Center. Residents of Nebraska can simply visit Unite Nebraska, or visit ne211.org and click the Unite Nebraska icon, to complete the form. Once the form is completed and submitted, a 2-1-1 navigator will work with the individual to locate the appropriate resource(s). These include needs related to healthcare, food, housing, utility assistance and more.

Unite Nebraska, sponsored by CyncHealth, is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us’ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health and health equity across the community. CyncHealth brought its vision to life for Unite Nebraska in 2019 to support health care organizations and community-based organizations to meet the social needs of Nebraskans. The intent is to centralize health and social care to ensure more efficiency in meeting needs and reduce duplication of effort. The United Way of the Midlands 2-1-1 Coordination Center offers centralized navigation across the state. The intent is to promote a “no wrong door” to addressing health and social needs.

“This program will expand the impact of the ability to meet social needs in Nebraska, empowering Nebraskans to access the social care they need,” said Dr. Jaime Bland, CEO and President of CyncHealth.

“United Way of the Midlands and its 211 Contact Center is honored to play a role in this key social determinants of health initiative,” said Shawna Forsberg, President and CEO, United Way of the Midlands. “It will complement our work around diversity, equity, and inclusion for health and other social services for vulnerable and under-served populations.”

The self-referral process kicked off July 25, 2022 and has already been accessed by community members. The intent is to ensure that the network can help meet the needs of as many in Nebraska as possible.
