State of Rhode Island and Unite Us partner to Coordinate Care

Unite Us is proud to announce the launch of RIServes, a statewide coordinated network in partnership with Rhode Island’s Office of Veterans Affairs (RIOVA) and the Institute for Veterans and Military Families (IVMF).

On Monday December 11th, Governor Gina M. Raimondo announced the launch of Rhode Island’s first coordinated network, RIServes. With the avid support and influence from Governor Raimondo, the RIOVA staff will operate the local RIServes coordination center to identify needs, and route clients to the most appropriate services based on eligibility and services. The network will use Unite Us to quickly match the client with a service provider and track the entire interaction to ensure quality and timeliness.

The launch of RIServes is a result of months of collaboration between RIOVA, local community providers and Unite Us. Over this time period Unite Us held strategy sessions with key stakeholders to define key objectives and priorities for the network, develop standardized workflows, and identify the network reporting and data needs.

For Unite Us, the launch of RIserves, signifies that the work we are doing both through building community networks and deploying our software into communities is in fact working. Receiving such strong support from the State of Rhode Island proves that this new model of coordinating care can change the way not only Veterans receive care but any vulnerable population.
